Where did the original deal come from?

Cleveland. Garrett signed a four-year, $160 million extension with the Browns last offseason, then had it modified in March to shift his option-bonus dates from the 15th day of the league year to seven days before the regular season. That earlier tweak set up the flexibility the Rams used to rework the 2026 compensation after the trade. The contract has now been adjusted twice in one calendar year without changing its headline value.

What's the AFC North wrinkle?

A poison pill protecting Cleveland. The trade sent Jared Verse and picks spanning 2027 to 2029 to the Browns, and there's a specific condition attached: if the Rams ever trade Garrett to an AFC North team, the 2029 pick Cleveland receives converts to a first-rounder.
